Catheter Care (♀)
“Hello Mr. Whatever my name is Grant, ill be your CNA today. How are you doing?” Grant flicked his eyes away from the mannequin to the woman with the clipboard to see if she enjoyed the name he had chosen for his “"resident”” She did not look impressed, he tried not to let it throw off his confidence. “Good good,” Grant sing-songed, pretending he had gotten a pleasant answer from the complicated hunk of plastic. “I am going to help clean your catheter today. Let me just close your curtain for privacy, gather some supplies, and wash my hands.”
The clipboard woman followed at a distance, writing something down as he used a towel to turn off the water. He hesitated in front of the linen cart, what did he need for this again? He should start with a barrier, something to put on the table. He grabbed the little blue chuck-pad and laid it out gently on the bedside table.
Grant turned his attention back to his supplies. He needed four washcloths, a second chuck pad for the bed, a towel for drying, and a bath blanket for privacy. His instinct was to hold them close to his body, but he was worried about infection control. You weren't supposed to hold the linin close to you when they were used, so you kept your shirt clean. But he was almost certain that in the real world his shirt would already be contaminated, he held the bundle at arms-length just to be safe.
There were more supplies, Grant took the time to lay out the washcloths and everything in a way that made sense, he had time. He needed the basin and soap, from the bedside table. But he might as well don his gloves now that they were right here. He placed the bar of soap gently on the workspace he had set up, on the way back to the sink.
He let the water run until it was warm, and testing that it was a decent temperature against his wrist. The plastic tub was surprisingly difficult to keep from spilling. not unmanagably so but Grant did have to focus on holing it. It was a relief to set the thing down on the over bed table. He took a deep breath still trying to shake the nervousness from him, glancing back to the instructor did not help his efforts. He grabbed the bath blanket and the chuck pad first.
"Ok Mr. Whatever, I'm back, I'm just going to place this bath blanket over you, it will help keep you covered and warm while I work." Grant moved forward gently unfolding the blanket, and placing it without swooshing anything over the dummy. "Good, now I'm going to roll you so i can put a berior underneath you to keep the bed dry." Grant rolled the pad as he spoke, setting the lightweight thing on the side of the bed. "I'm going to ask you to come my way just a bit" It was easy to scooch the plastic form towards himself, before proping the closer knee up, and making sure there was enough slack for the catheter cord not to be pulled. "Ok now we're going to roll away from me, ready one, two three."
"Here." The woman watching set down her clipboard on the bed he wasn't using, and placing a steady hand on the dummy to keep it from moving.
"Thank you." Grant offered a pressed smile to show his appreciation, before carefully flipping up the blankets so he could get the pad underneath the dummy's ass. "There we go, Mr. Whatever. Now I've tucked the pad under neath you, there's going to be a bump when you roll onto your back, ok?" Grant let a brief second pass before rolling the mannequin back towards himself.
"Great job, now me and my assistant here are going to switch places so we can get that bump out from under you safly." grant laid back down the blanket to keep the ""resident"" covered and then moved quickly around the foot of the bed. "you still doing ok Mr. Whatever? no new pain or discomfort? good good." grant said as he got into his place on his new side.
"I'm going to bring you back my way," Grant said, before moving the plastic person closer to his side of the bed. "ok, we're going to roll the other way his time. Again on three. One, two three." The dummy turned easier with the woman there already ready to support it. and grant could easily switch to turning up the sheets, and unrolling the plastic pad he had put on the bed. "Back to center." He said, and the doll was rolled back.
Grant took a step back collecting his thoughts again. "Let's move some of these linens around. I've got the bath blanket over top of you for privacy but I just need the sheet and gown underneath out of the way so I can get to you while im cleaning." Grant reached under the thin blanket, to pull the flat sheet down to about the dummy's knees. and folding up the hospital gown at the waist.
"You can pull down the bath blanket now." Grant looked up to the instructor, nodding before turning the bath blanket down too. to expose the private aria. He turned back to the bedside table with supplies, rolling it closer to the bed, before picking back up the basin and setting it on the side of the bed.
"Ok Mr. Whatever, i just want you to check that the water temperature is good for you." There was another pause where Grant assumed that both humans were pretending that the doll could actually have an option on the water temperature. "very good." The plastic bin went back to the table he turned back to place the towel across the mannequins thighs. Grant dropped in his four wash cloths. into the warm water. and then drawing one out to rub the bar of soap into
"im going to start by cleaning your catheter tube." Grant reached down, grabbing the tubing right where it came out of the dummy, and using the folded washcloth to wipe down and away from the opening it came out of, three times, turning the cloth to use a clean aria each time. "Good ok, now I'm going to rinse the tubing." He turned and set the soapy washcloth next to the basin. before repeating the same steps with an unsoaped cloth. "And now dry." He finished, using the towel to pat at the plastic line.
"Ok, now I am going to clean up your bits." Grant mumbled unsure if "bits" was the right thing to say here, as he put soap on his third washcloth and folding it into fourths. he started from the inside, using a gloved hand to spread the labia. and wiping from front to back with the first corner of the washcloth. he let go of the labia and folded back the used corner of the washcloth, wiping both of the labia-majora with new corner of the wash cloth. and then one of the groin aria with the forth corner, and then the washcloth was folded into a little triangle so he had a clean spot to wipe the other groin aria.
"There. Now I'm going to rinse you." Grant pulled the last washcloth out, and repeated the motions to wipe way the soap with a wet cloth. "And dry again." He set the final washcloth back on the table, and then gently patted the ""residents"" """genitals""" with the towel still moving down and back again.
"Ok," Grant said it for himself regathering his thoughts, already moving to cover up the exposed area of the fake person. "Ok. I want to get that check pad out from under you, so we can roll back away from me." Grant pushed the thing gently back onto its side, fucking the pad in a line across its body. "Good, now back to center." Grant looked back to the instructor, still half doping she would give some indication of how he was doing. "Can we switch again?" the woman nodded, and both of them switched sides again grant making quick work of rolling the Maniquin over and removing the pad.

1 Greet resident, address by name, and introduce self? (IC)
2 Provide explanations to resident about care before beginning and during care? (IC)
3 Wear gloves while handling the urinary drainage bag, graduate, or any urine-filled container, and remove gloves before documenting I&O?
4 Set graduate or bedpan on barrier placed on floor and empty full contents of drainage bag into the graduate or bedpan?
5 Empty contents of urinary drainage bag without contaminating drainage tube (e.g., touching container) and close and protect drain (e.g., clamp and tuck drain into pocket) after emptying drainage bag?
6 Set graduate on flat surface protected with barrier to read, obtaining measurement by reading graduate at eye level, and if urine poured into graduate from a bedpan, complete task over toilet pouring the full amount of urine into the graduate?
7 Empty urine in graduate into toilet after measuring, rinse and dry container, pouring rinse water into toilet?
8 Record output with clean hands?
9 Record output within +/- 50 mL/cc's of nurse’s measurement?
10 Record output as urine and indicate the correct time on the I&O form?
11 Leave bag hanging from bed frame (not side rail), and drainage bag and tubing off (not touching) the floor?
12 Keep urinary drainage bag positioned lower than bladder throughout care?
13 Complete skill having stored equipment, placing soiled linens in hamper, and disposing of trash?
Occupied bed change
1 Greet resident, address by name and introduce self? (IC)
2 Provide explanations to resident about care before beginning and during care? (IC)
3 Keep resident positioned a safe distance from the edge of the bed at all times?
4 Remove and replace bottom sheet on one side of the bed, before turning resident to remove and replace sheet on the other side of the bed?
5 Keep resident positioned on a bottom sheet throughout procedure?
6 Secure bottom sheet to mattress (e.g., for fitted sheet secure over all four corners of the mattress; for flat sheet, tuck at head of bed and on sides and extend toward bottom of mattress so that resident’s heels are not against any exposed mattress)?
7 Leave bottom sheet free of creases or folds?
8 Turn or position resident to remove or replace sheet(s) without pulling sheets in a manner that creates friction and risks skin shearing?
9 Replace the top sheet over resident with a clean sheet?
10 Tuck top sheet under foot of mattress leaving sheet placed loosely, avoiding pressure against toes and allowing for foot movement?
11 Leave top sheet placed on top of resident to cover body up to shoulder level, without tucking in along sides?
12 Keep pillow positioned under resident’s head throughout and at the end of the procedure, except when removed briefly to replace pillowcase?
13 Complete procedure with resident positioned between the top and bottom sheet?
14 Complete skill placing soiled linens in hamper and disposing of trash?
15 Ask resident about preferences during care? (IC)
