Max is in some kind of realm. Everything is dark and there is only one light at the end of an imaginary tunnel. She is following it, until she crashes into someone. This someone is Billy and she just hugs him. He hugs her back, hesitantly. Before she can say anything, he complains about the letter for him (probably something like: "not with this cheesy shit next time"). After that he holds her at arms lengt and pushes her away full force. Everything is turning blurry and she screams his name, but the only answer is: " Don't you dare coming here so soon again. Your time isn't up now".
The end of the scene is Max moving for the first time in the hospital (because she is kind of back in her body and mind, if you know what I mean...maybe).
Okay this would be good fan service because of Billy and we would get a little bit of aggressive caring/ explanation for her nonresponsive behavior (because she is literally not there, which could also mean that she is in some kind of mind cage Vecna set up, but I wanted it to be a little better than just a psychotic dead end, thank you very much). Even though seeing someone being alive from the death again like this wasn't really a concept until now (if I remember correctly), the aspect of talking to them like Max did in her letters was a step in that direction. Therefore this would connect really well and everybody wants a reaction from Billy for that letter, you can't tell me otherwise.
