Suo’s instinct responds with sharp, longing want. As though his body thinks that Sakura’s hands in his hair and Togame’s arms around his waist are a cure for his symptoms as definitively as it would crave water during heat stroke. His face runs hot.

“Suo?” the doctor prompts.
“No. No one,” he lies in a thin voice. “Can I try the medication?”

Summary
On one hand, being asked to look out for someone else’s sub is really weird request. And he is concerned that Chika might change his mind and decide to try to beat him into the ground. There’s also the fact that Endo doesn't really seem to like him all that much.
But… it does prick at his conscience. Endo really did look pale and tired. It can’t be good from someone’s instinct to be constantly dysregulated. The more he thinks about it, the more it feels like the right thing to do.
And well, Umemiya has always been a little bit irked by his inability to win over Endo. He considers himself a likable person; even Chika enjoys his company, of all people. Ume doesn’t know why Endo never warms up to him no matter what he does.
Maybe he could just…try? All his friends make it look easy. It’s instinctual, right? It can’t be that complicated.
