No amount of preparation could have prepared him for what he saw on the other side of the door. Mr Stark is sitting up in a hospital bed, pillows behind his back and head, eyes closed and face looking relaxed. There’s an IV bag hooked up to his left arm that’s resting on top of the right arm. The right arm that had been amputated. The one that now looks to be a mechanical arm kinda like the one he saw in Germany, but much more...sleek? Peter isn’t sure what word he’s looking for.

“Peter, Peter, Peter,” Tony chided lightly. “What are we going to do with you? You clearly don’t know how to behave. You know, if you had just listened to me, we could’ve avoided this. I would have let you keep the suit, Spider-Man, everything. That was Plan A, but you ruined it. Now it's time for Plan B."
Even with his head down, Peter could sense Mr. Stark rustling around in his pockets for something. If he trained his ears towards the noise, he could just make out a thrumming vibrational sound, mixed with an incessant buzz of high-pitched static.
Peter threw his head up with panicked realization. He knew that sound.
“You’re not Mr. Stark,” Peter managed to gasp.
Mr. Stark smirked. “Aren't I?"

Summary
One day, Mr.Stark starts acting strange. Peter doesn't think anything of it, the man was peculiar to begin with. But as days pass, he starts acting weirder and behaves in a rather questionable manner. Everyone notices and they try to put the puzzle pieces together bit by bit.
Mr. Stark does nothing but hinder this process. Eventually, Peter realizes that they've been trying to complete the wrong puzzle set all along and all the pieces proceed to draw a very different conclusion.
