Steve never gets on the plane. He never meets Catherine, and that final conversation between them never happens. Consider it deleted entirely.
Instead, the story picks up a week and a few days after Danny returns home from the hospital.
From there, we get to see what might have happened if Steve had made a different choice—if, instead of leaving Hawaii behind, he stayed.
Because sometimes the ending we were given isn't necessarily the ending the story deserved.
