What if Madara had noticed his slow descent into madness and realized something was wrong? What would have happened if he went to get a second opinion from an expert? What if Madara had more support when he was struggling?
Or: Madara still left Konoha and tried to destroy it, but it was never what he wanted. Tobirama just wanted to save the love of his life but failed to do so alive. He wasn't allowing the same thing to happen even if he was reanimated. Madara wouldn't leave him again.

Team 7 didn't make it in their fight against Kaguya in different dimensions. So the Sage of Six Paths grants reanimated Hokage a chance of fixing it all before it started, but only one can go. They chose Tobirama Senju because he knows Uchiha better than anyone else and has the best chakra control over all of them.
When he woke up in his 9 year old body next to his long dead twin, Tobirama swore he'd prevent Kaguya from coming back. And for that he must watch Madara closely in order to catch Black Zetsu and deal with the source of the disaster himself.
And what's the best way to closely watch someone from their enemy clan if not marrying them?

He’d tried to explain this to the official who’d summoned him out of bed just before midnight, tried to explain that they ought to wake Umino, or the Sandaime, or literally anyone else, tried to explain that Naruto had only been his genin for a few months and that really, he didn’t know her that well. All to no avail, and now he was standing in the office of the shinobi headquarters that had been converted to the new law enforcement station after the Uchiha police regime was wiped out, watching through one-way glass as the student he was desperately trying not to get too close to rocked herself, arms wrapped around her knees with a shell-shocked expression and wide, distant eyes.

Even at the moment of the hiraishin's conception, Tobirama knew the dangers of meddling with the very threads that make up the fabric of existence. He knew that repeated usage only made it easier to traverse between the dimensions because the user became physically more susceptible to slipping through the cracks.

Summary
The Hidden Leaf Village in Naruto's time seems to have forgotten just why the village was founded and have lost their way. Through a mishap with a hiraishin seal, Tobirama finds himself during Naruto's time and he is having none of this nonsense. His brother may be gone, but he won't stand to have Hashirama's dream trampled so.
And if he has to cause a civil war or burn the village down to the ground? Well that depends on how much resistance he meets.
All Tobirama does know for certain is that no child will suffer underneath the protection of Konoha and if he has to take them in and care for them himself, then he will.
