A trepidatious voice calls out from the other side, “Should I call 911? You’ve been non-stop vomiting for, like, an hour.”
“Just a second…”
Nagi begrudgingly hoists himself up, trying to keep his head from spinning. There’s a wicker laundry basket in the corner, which he tosses the suit into and frantically shuts the lid. The knocking won’t stop, so Nagi races over to cautiously creak open the door and face his friend.
Now revealed, Reo looks extremely worried, holding two loose Zofran and a large bowl that’s way too expensive to hold puke. He’s not, at least, surrounded by a dozen reporters and equipped with a microphone. So he’s not suspicious, Nagi actually pulled that off and—
“Holy shit,” Reo’s jaw drops and he gasps. “What happened in there?!”
A hand moves up and grazes a tender spot on his forehead. Nagi winces. So his quick trip to East 6th Street had come with a surprise souvenir. What a pain.
“I…” Nagi begins to feel stupid for not glancing in the mirror beforehand. “I slipped and hit my head… on the toilet…”

Summary
Mikage Reo opens his eyes to find himself in a place he's never been before, and for some reason, he looks no older than seven years old. Is this some kind of dream, no, nightmare? And why is he wearing such frilly pajamas!?
Reo's maid tells him that he is the Duke's son and is engaged to the Prince. To Reo's chagrin, he now realizes that he is in the world of the Dating Sims game that he used to play. The game in which Isagi Yoichi is the protagonist and Nagi Seishirou is one of the capture targets aka love interests. And Reo, of course Reo is the villainess of the story.

I think I saw a couple of stories like this in Nagireo works but lemme add one more. Reo is a guy I know but indulge me and let me use the word villainess XD -

He looked at their opponent, then he looked at their ranking. They were number one, they never lost a match. Nagi and him, they were unstoppable.
His gaze fell on Nagi Seishirou; the boy looked as uninterested as ever. His figure was relaxed and Reo could read his lips murmuring ‘ah, I forgot to do my dailies today.’ After this match, Nagi was going to awaken his monster. He was going to abandon him.
The thought didn’t bother him anymore, not like how it did the first time. But the pettiness was gnawing at him. When he opened his mouth he spoke like a bitter, unhappy man.
“Let’s relax, guys. Why don’t we lose this match?” He smiled at his teammates, but it didn’t reach his eyes. “Think of it as charity.”
