Summary
Dazai lies on the table beneath the bright medical spotlight, a shining beacon in the otherwise dim surgical suite. The lights wash him out, making him look translucent and sickly. His blood-soaked coat lies sopped and deflated on the table, outlining his slim form in a way that almost makes him look skeletal.
And Kunikida realizes that he’s staring head-on at a future Dazai has been sprinting toward for as long as Kunikida has known him—a future where his corpse lies lonely on a morgue table, empty and lifeless.
Or, one year into his employment at the Agency, Dazai hides an injury from the team until he collapses. Kunikida helps Yosano stabilize him and is horrified by some of the things Dazai lets slip and the scars he finds beneath Dazai’s bandages.

Summary
Our dearly beloved Atsushi from canon and our deeply hurt Atsushi from BEAST switch places. It's less like accidentally walking into a circus and more like being violently drop-kicked into a burning swamp. With mutant alligators. And trauma.
“Atsushi-kun, are you alright?”
That is a funny question for all of two seconds before he registers the name. Atsushi-kun.
He almost chokes on his own spit. The Agency people stare at him in concern, like they're confused to see him unwell, when he’s definitely attacked them before. His senses are in overdrive. The infirmary has three possible exits and he has a clear view of all of them—he's cornered in which isn't the best for movement but good for sight—and then he moves on to an attack plan because no man calls him Atsushi-kun except for Dazai, and the man staring back at him is definitely not Dazai.

"Generally speaking," Dazai says, and he doesn't sound anything like the Dazai that Chuuya knows, "I can't imagine I'd go through the effort of faking my death when dying is both easier to achieve and more of a pleasant experience. Though it depends on the how and the why, of course."
"Bastard, stop talking in circles and–"
"Boss, huh?" Dazai says, and Chuuya breaks off. It sounds wrong, from Dazai, and Chuuya lets Dazai go. Dazai drops down to both knees, and then sits there, gazing past Chuuya and looking at the makeshift shrine on the imposing wooden desk. There's something dark in his gaze, and it's almost comforting. Almost, because it's familiar; it's a knife in the darkness that Chuuya has watched bring down hundreds of people.
"What," Chuuya snaps. "Going to tell me you want the position back?"
"Back," Dazai murmurs, drumming his fingers on his knees before he finally climbs back up to his feet. His hands go into his pockets, his posture relaxed and easy, and none of it makes any sense. "Chuuya, was I really?"

While Leon's Spanish – as proven before with the Don Quixote debacle – wasn't the best, he was pretty sure he’d understood what Luis meant.
