Sukuna died and got reincarnated in a world where curses don't exist. Getting used to this new body and weird instincts is one thing. Acknowledging his intelligence and sensitive smell to make him a diabetic-alert dog is another. Then an unexpected yet familiar person comes to his training center, and Sukuna is suddenly very determined to become their partner.
Or: Sukuna is a service dog, and Yuuji has diabetes. The first one needs to help survive the latter. Ironic, isn't it? Not enough. Let's make them very attached to each other in the process!

“I think it’s perfectly acceptable that I’m untrusting of a demon,” Megumi responds, cutting him off.
The grin Sukuna gives him in return simultaneously makes his blood run cold and lights a fire in his veins. “One of the many things I appreciate about you.”
Megumi pauses, thinking, wondering if he really can trust a demon of all things, let alone one claiming to have been following him and protecting him for years. Still, he can’t deny the string of coincidences that suddenly make sense, and he certainly can’t ignore the fact that this demon did just protect him. “If you really are my guardian demon or whatever,” he says, stepping out of the alley and onto the street, “then get back to work.”
Megumi can’t see the demon’s face but he can hear the amusement in his tone when he answers with: “Anything you wish.”
