He's back from Hell, but he doesn't belong. Not here, not out. Everything that happened down there, what he did... He can't talk about it. And even if someone hears, they won't understand. Not if they hadn't been down there and seen him.
Seen what he'd done. Knows that he still deserves to be there. But also, somehow, believes that he deserves to be saved.
Not unless they're one particular being. Who can somehow appear when thoughts are manifested as ... longing.
(Or, when Dean being with Anna in the Impala turns into Dean longing for ... something -someONE- else.)

(This isn't said out loud to both by their angel buddy, but Cas had alluded as much to Dean, so he knew. And passed just enough information along to Sammy, as the oldest needs to keep a few things to himself.)
It's the year 1692, and in the middle of winter there's been a witch outbreak. Well, that's what everyone says, in Salem Village and general vicinity around (the surrounding colony of Massachusetts Bay).
